I. Introduction Health inequality has become prominent on global policy agendas as researchers have come to regard average health status as an inadequate summary of a country’s health performance (1,2). Almost all health inequality studies have documented differences in average health status only across groups of people. Those with an economic focus have measured differences in average health status across income groups (3,4). Researchers with a sociological focus have examined inequalities across social classes (5,6). Other scholars have focused on differences in average health status among racial or ethnic groups or by educational attainment or occupation (7-15). Demographers have long studied differences in average health status, particularly in children, across age, sex, education and racial groups(16-18). And most researchers consider differences across political entities such as countries or local governments. This paper complements the group difference approach with estimates of health inequality from what may be viewed as more of a purely public health perspective. That is, we document basic levels of health inequality across individuals because public health policy should be targeted at reducing inequalities across individuals, in addition to reducing disparities in average health status across groups in society.
